A revolutionary product has emerged that is improving the way breast revision surgery is completed. It is called acellular dermal matrix (ADM), and it works as a reinforcement shield to correct some of the most common reasons for breast revision surgery, including capsular contracture, bottoming out, symmastia, wrinkling and rippling, and fold malposition. It is designed to reinforce weak or inadequate tissue and support the growth of the patient’s own tissue. Introduced in 2008 as Strattice™ Reconstructive Tissue Matrix, ADM has gained acceptance among U.S. doctors to be used in breast revision surgery as well as complex breast reconstruction cases.

ADM is made from porcine skin that has been purified. Porcine skin has been used for many years in the United States to repair and replace damaged tissues, such as heart valves. For breast revision surgery, ADM comes as a pliable sheet that is sutured to the tissues that surround the implant. Once the patient’s blood vessels grow into the ADM, it will have its own blood supply and will become a permanent part of the patient’s body. Allergic reaction to this product is not possible, and infection is rare with ADM. This product seems to perform extremely well in preventing the recurrence of capsular contracture, which is the formation of scar tissue around an implant. When this occurs, the breasts can feel firm and painful. In some patients, the scar tissue can even recur once it has been removed. For such patients, ADM would be a tremendous benefit.

ADM is believed to prevent scar tissue formation because it promotes cell regeneration for wound healing and prevents the inflammatory process. Since it can also act as a tissue “buttress” to hold the implant in place, it is ideal for women who have implants that have malpositioned due to weak breast tissue. Malposition can recur in 40 percent of patients during revision cases where the patient’s own tissues are used to hold the implant in place. Malposition is very rare with ADM. This product also disguises implant rippling in patients who have thin skin.
